Think Chernobyl, train derailment, boat ballast tanks flooded, all green lights at a 4 way or venting of chemicals at too high of a rate etc. Something that should not be AI but someone did it any ways to save on hiring a few people or writing the code. The most likely way you will die to AI? You need some sort of medical whatever and the AI flags you as not being covered by your insurance. Maybe you finally bash your way to a solution after months of phone calls but by then it's too late or it's just no, we don't cover that now.
