Last night a medevac helicopter crashed seconds after take off from the eastern edge of Catalina Island shortly after sunset. The aircraft was a highly advanced dual engine H-135. Even in the rare event of an engine failure, the helicopter should be recoverable. No one’s saying it, but there’s a very high chance this was spatial disorientation over the pitch black ocean. Pilot probably dipped the nose down too far while accelerating and didn’t realize he was sinking until they went splat into the water. These pilots are supposed to be the best of the best and yet all across the industry you’re seeing these professionals with years of experience making completely bone-headed mistakes. The Vietnam 787 where the 64 year old captain was pressing the brakes inadvertently on take off nearly killing everyone onboard is a recent example. The Amazon 767 overrun in Miami with the pilots touching down roughly 3/4th of the way down the runway without even having the aircraft setup to land is another.
