Both the pilot and the copilot are responsible for things like turning on the ice countermeasures BEFORE entering a storm. She says they're managing the ice, but they clearly aren't. They obviously didn't turn it on until they hit the storm. It didn't have enough time to warm up and it didn't prevent ice from forming.
