A case of pathological phimosis is either as you said a frenulum breeve - a tight banjo string, and/or a fibrotic ring of hardened tissue in the foreskin. A frenulum breeve can be fixed with a frenuloplasty or in worst case a frenulectomy. With a fibrotic ring in 80-95% of cases it can be corrected by combining corticosteroids and stretching to completely fix this ailment completely. Don't unnecessarily compromise the frenulum (all circumcisions remove parts of it or it entirely) as it's considered as pleasurable if not more than the prostate.
