It only recently got mainstreamed by the whole "-maxxing" thing. Caring about how you look (too much) used to be distinctly Gay. In fact that was how you'd be insulted if you were too vain for several decades, from the 60s to maybe the mid 2010s if you showed too much interest in superficial shit like your hair or your body or your skin or your weight, that would be received as a gay-coded behavior and people would call you vaguely homophobic names like "prettyboy" or whatever. Nowadays looksmaxxers are straight-up wearing foundation and getting buccal fat removal surgeries, but you couldn't do that shit in the 2000s and have people know about it. It was a fringe movement people referred to as "metrosexual" for a little bit but even then they were mostly just made fun of. Putting as much effort into your looks as women do is a relatively new phenomenon but for whatever reason was always primarily a gay thing first.
