If you're in shape / get enough sleep / read a lot / etc. I think it's probably fair to say you will in practical terms be 5-15 IQ points more intelligent than you would be otherwise, but that's more like "making full use of what you have" rather than "making your brain bigger"

I am noticeably less intelligent than I was at late teens / early twenties peak. Which is fine career wise because I am established and didn't need that much horsepower in the first place in practice, but I was (am still maybe for my age? not sure) top 1-2% and it was core to my identity when I was younger and having it slowly go is horrifying.

There are a lot of things you can do to grow more intelligent. I know you won't do them though, because you're already stupid and you're lazy, so there's no point in talking about it. The initial state of your brain is defined by your genetics. The rate you can learn new things peaks in early childhood and has a smaller peak in adolescence. The rate that you can learn new things starts to decline rapidly once your brain stops growing at 25. All of these rates and peaks are defined by your genetics, early childhood experience, childhood diet and exercise habits, and any brain trauma you may have suffered.
