In engineering, accidents happen all the time but on a small scale usually, big accidents usually dont have many people involved these days due to safety, but sometimes people cut corners..Here's an example of it happening at tamu since someone posted that school earlier.

This liquid nitrogen tank, normally a non explosive inert gas, shot through a 3 storey building, if anyone were in the way they would be instantly killed, but thankfully it happened at night with no students around. Why? because the pressure relief valve had been welded shut to prevent leaks. Just an example of how fixing one problem can lead to a new problem if you're not careful as an engineer
