I am at a lower-tier newish MD school and am about halfway through my clerkships. When I got into medical school I found out my dad was diagnosed with Parkison’s, so I ended up going down a crazy route of wanting to become a neurosurgeon and did a bunch of different research things and so far I have 5 papers, one first author and another 5 in progress.

I don’t really want to do neurosurgery anymore now that I have processed a lot of things, and I am torn between a lot of different specialties. My number one interest right now is orthopedics, however I am torn between IR, EM, and neurology as well.

I was offered a paid research year at a well known orthopedics program when I reached out about my school not having a home program. However, I am unsure if I should accept this offer as I feel like I have not been performing well in my clerkship grades.

For context my school has an honors / pass system where <~20% generally get honors. I have received a P for surgery and a P for OBGYN despite performing >95th percentile on the shelf as the grades are a composite of evals and shelf scores, with a harsh evaluation system.

I am scared if I take the research year due to my poor grades and relatively unknown school that I could end up still not matching into ortho. Additionally, since I was so focused on one specialty in the beginning I may be missing out on other opportunities of specialties I could enjoy.

Any advice would be greatly appreciated.

You should ask what is required of you to get better clerkship grades from the ones evaluating you. Not out of a place of like grade grubbing or brown nosing but like if you sincerely express interest in excelling and work with the people evaluating you, you might have more success? Med school is brutal. Get that honors score.

You said it yourself, just be an OBGYN surgeon and do C sections all day long. Easy work for good money.

I am a psychiatrist in private practice and I love it. Originally I was going to do anesthesiology and I am glad I chose psych instead. Low cortisol as fuck.
