Corpse smugglers.

Necromancy is rampant, the elites almost all do it, returning lost family members back to life. But you can't be resurrected in your own original body, only shoved into a foreign body. So people are killed for their resemblance to certain elites, their bodies sold off to fuel their intermittent but otherwise unending lives.

Elites thus engage in a proxy war by denying their rivals fresh bodies to assimilate into. Thus the pressing need to acquire appropriate corpses for resurrecting your own family, to use as foot soldiers for conflicts with other families, and to stop rival families from doing the same.

Corpse smugglers, grave robbers, body thieves; all of these will gladly divest others of their physical bodies and return them to their necromantic masters. They're even paid to harvest particularly high quality body parts so the nobility can work on Doctor Frankenstein-esque super soldiers.

That's not a pressing need though and elites are the least likely to want to resurrect family anyway. Even modern petite bourgeois are infamous for the squabbles that wealth causes. Never mind historically where elites at the top of power structures were assassinating or imprisoning their relatives left, right and centre so they could have the power and wealth.
Why would you ever want to resurrect your competition? and if you did why would you care about likeness all that much?

It's a cool plot point but it doesn't make sense at a big scale unless your world is particularly noble;

A). The same processes that allow for resuscitation allow for modifications to make some pliable. Hypnotic suggestions can be made to enforce loyalty to the family.

B). The reason the elites are the most likely people to do it stems from the fact that they have the clearest records of who and what exactly they're bringing back with any bit of necromancy. If Billy Bob doesn't really have a good idea of who his great-grandpa is, then he doesn't have a shot in hell of bringing him back.

C). Frankly, I fucked up when writing that. Most of the people properly resuscitated through this method are going to be servants of the family, their minds bound in much the same way as the family's resuscitated own. People work for these families as a way to achieve a sort of petty immortality.

D). Much of the elite's power structure comes explicitly from being able to pull a millennia of skill and abilities out of their ancestor's mind. In this case, then the elites typically just essentially command their ancestor's shade to just give them the information or temporarily merge with them to access said information directly.

Narcos are narcos. A favela basically got transplanted onto there Forgotten Realms-style and split off into a group that tried to integrate with the previous inhabitants and eventually found the One True God to act as His enforcers, while the rest managed to adapt and restart their business with a major warlord state who just so happened to be led by terminal addicts being their main partner.
The former assumes the latter died off, but are in the process of discovering the true fate of their counterparts while working in tandem with a third party to dismantle the warlord.

I've had a rough equivalent to a mafia-esque organization in the country My PCs have played in for the last couple years for a while now, but never gotten a chance to really engage with it since they're not a super big deal outside of one particular province. Their main things are guns and the setting equivalent of Welsh gold.
