Except for the tiny sliver of exceptional individuals all normal humans in RPG fantasy worlds live implicitly under the constant threat of being horribly killed by various superpredators or subjected to a fate worse than death by malevolent entities. Only begging for the grace of your superiors can save you. This dynamic plays out all the way to the highest levels of reality where the only thing that prevents the evil gods from torturing everyone forever is the intervention of good gods. Likewise a village is powerless against the many, many horrors that fill the Monster Manual unless a nearby group of heroes are willing to put their lives on the line to protect them. Of course if the heroes decide to exploit and abuse the villagers nothing but another group of heroes can save them.

the "peasants work less hours" is a pop culture myth
peasants worked roughly the same amount of time a modern wage slave does, its just more lopsided with a lot more work being done in some parts of the year compared to others
but peasant work was much, much more physically demanding
when you factor in obligatory work to the lord and non-farming related work like repairs, they can easily exceed modern work hours while having much harder work for the same hours

Actually chew your meal, fast for at least 5 hours between meals, avoid drinking liquids with your meal and for two hours after the meal and fast at night. Do that for a couple of days and see how much toilet paper you actually need. It's what I do when I go forest camping and it works like a charm.

I 've seen that argument and they usually factor in stuff like cooking and repairing/cleaning your house or raising your children as work and that seems preposterous to me. And yes peasant work was much more physically demanding sure but that promoted better physical (and mental) health. I 've seen grandmas in their 80s that lived like peasant workers that can still trek 10km of mountain passes daily while a 60yo with a desk job can barely use the stairs or get in a car that is not a raised up crossover.

What I would miss dearly in a faux medieval setting without or with limited magic is antibiotics and tetanus shots. A minor accident or an altercation with a wild animal could quickly turn fatal.
