The Emperor was the first human/Perpetual to manifest psychic powers on Terra and by far the most powerful. Before the Emperor, there were no psykers on Terra.The Emperor and the rest of the Perpetuals attempted to lead humanity from the shadows, but the Emperor was a control freak and sorta a tyrant, so one by one the Perpetuals, except much later Malcador, turned on him or abandoned him.

Malcador still refers to Big E as the First Shaman, and it's confirmed he originates from prehistoric Anatolia as per the old version of the shaman fusion dance. With the reveal that all the Primarchs are artificial Perpetuals created by fusing powerful warp entities from human culture, it lends credence to the idea that the Emperor himself might be the first artificial Perpetual, created by the Perpetual shamans before history using their suicide ritual to combine their souls. It would also explain why the Emperor is leagues more powerful than all the other natural Perpetuals like Malcador, Erda, and Ol' Pious.
