The expectation is that at least the mayor or magistrate would understand what a paladin is, and that these sorts of miracles are only granted to the truly virtuous, and that they can also detect evil. But honestly, I agree with you. There are just too many different supernatural things. 3rd edition told us that people can't even tell the difference between wizards and warlocks (let alone sorcerers), so surely a paladin is at the very least indistinguishable from a cleric, who can have any sort of ideology. Frankly, Dragonlance made a good case even before 3rd edition that most people can't even tell the difference between divine and arcane magic. That's part of why I prefer the 4e model that lets a paladin be a champion of any faith and whose behavior doesn't affect his powers, or the 5e model that says a paladin just needs to be devoted to SOMETHING. You can still have a traditional paladin, especially in 5e, but it also expands the concept in a way that I, at least, find interesting.
