I'm generally not a fan of mechanics that dictate how my character feels about something. If I decide there's no trauma, then there's not. If I decide they don't care about Lovecraftian insanity bullshit, then they don't.

That depends on how much of the game is built around those mechanics. Personally my limit is things like Intimacies in Exalted or Nature & Nurture in WoD. You see a lot of retards talk about player agency, like that faggot who tried to make storyshitting a meme, but there's a difference between not having many options for what actions you can take in a given situation, or that the next town the group reaches is the village of Sheepfuckershire no matter what direction you go, and how your character feels about something. That is the only player agency that really matters.

Usually if there's a track for intimacies, then it's less about (you) being attracted to the NPC but rather how much the NPC is attracted to the PCs. I'm not familiar with WoD or the other aforementioned, but that's the general common sense on how DMs use those tracks/clocks, because most know they can't make players feel something they don't or play the game.

In all the PbtA or GURPS games I've played, when players have additional tracks or session specific tracks for things like Sanity, it's treated as a resource denoting what additional abilities they have access to or possibly places they can explore.

The brain of a creature that only exists in 4-dimensional spacetime simply can't healthily absorb data from something that exists in 5 or more dimensions. I'm finding that I prefer something like Fear Itself's stability to CoC insanity. Stability is easier to get back until you hit a certain threshold, so it's less of a death spiral than sanity unless you get hit over and over. Trail of Cthulhu even has sanity as a separate track specifically for truly world shattering revelations.

This falls apart because you don't just lose sanity in CoC from knowledge or experience of the Lovecraftian, you also lose it for "mundane" things like seeing a bunch of corpses or getting shot at.

In Exalted Intimacies are the specific things you care about the most, and there are a few things that interact with them mechanically, such as mind control, and doing things that satisfy one of these attachments can net benefits. In WoD, Nature is your actual personality, Nurture is your facade, they're just a roleplay guide basically but you have to pick from a defined list.

And if I decide my character simply does not give any extra thought to the non-Euclidian squid monster or worry about trying to make sense of it, and instead just throws his molotov and hoses it down with an M-60, the game should be forced to sit down and shut up about it. If we're talking about one of the things that has actual psychic abilities and it's alien thoughts are literally intruding on the character's mind, that's different, but "looks weird" shouldn't be something I'm required to care about.

Okay, then the question is who defines Intimacies, and that's the player. It's analogous to backgrounds and histories in that that Players themselves are asked to choose what they care about and present it forward as both ways to display their motivation or as levers for exploitation. It's like saying you love your dog, and when (the GM) it's killed by the mob, do you go on your vengeance fueled adventure. If you don't, that's fine, that's on you to choose what action you want to pursue. It's that Trey Parker, emotional investment... therefore it calls you to action.

The GM doesn't track or foist those relationships upon the player asking you to care, he's just testing whether you actually care enough to act.

I really like the idea of a DnD or Pathfinder clone with a horror/dark setting and a sanity score.

Imagine Scoob and The Gang vs.Tremors, but its a dragon and the party vs purple worms and everyone has lost some marbles well before this dumb shit ever happened. Now it's getting worse.
