Start with completely different planet and cosmology. The crucial thing is mever to have any parallels with our world, history or religions. If something reminds you of a real world counterpart, you must completely randomize it. The trick is to either play in our reality or in something entirely alien. Never mix our world with fantasy or sci fi.

I've got the type of imagination that feels more like I'm just describing real people and places and the details auto-generate. So I'll just make an outline and some outlines within outlines, and just let it happen. If that sounds insane you have to understand that even when I do plan things out more and write lots of detail then when it comes to the game it just gets overwritten as things play out. I can't seem to help it. And no one in our group sees any difference between these two methods. The tradeoff is my games have more wild flexibility and can go more interesting places, but my friend's game has had some crazy intrigue and plot twist, amazing foreshadowing, and most different he drew a whole map for the setting.
