They started following a translated AAR of the original PS Vita version of DR1. Big reason why those games were ever translated to English.

The world is a literal one, it really exists. It can be symbolic of other things, but that doesn't mean it is all a fantasy. Hugo is a sick child who has the power of creation post some apocalypse. Hugo creates representation of his mother who, do to being neglectful by nature, isolates Hugo and rules the world with the guardians. Forgotten, Hugo makes The Batter, a representation of his want for death and his dad (who he has loosely negative feelings about) to put him and the world out of their misery.
