But then I got an altar that took my active item and stacked it, giving me +3 to the potion, so does that just mean it healed more?

I was stacking pain gloves on Shimiao and noticed that they barely worked. They definitely did not damage her every hit and sometimes they would do no damage until she healed to full.

Void items were at least a smart way to cycle less desirable stuff into something more interesting, like transforming Fungus to Wungus and making it one of the most rock solid items in the game. I wouldn't mind something like that for Alice.

I think it was a case of Hopoo thinking "Isn't this so fun and wacky" where you just groan as you have to deal with more bullshit. In contrast, I think Alice does a better job of alt builds. Like the fact there is an actual build around jumping and being in the air goes a long way. The only thing I think both games struggle with is making a true tank build though thats because the tanking items aren't great in either game.
