My guess is that the world is a manmade object floating in space. Life survives on the inside of an spherical supermassive ship with artifical suns at the center. The ship was originally created by an ancient society to escape some calamity that affected their planet, but almost everyone has forgotten this. The story is an allegory for Noah's Arc and the Garden of Eden at the same time. The protagonists' final goal is to escape their world and colonize a real planet for some notion of freedom and a new life.

The series being open world means that for every game there needs to be a totally new world or reasons why the old one changed. Perfect Works takes place over hundreds of years so time could explain why land masses change, but Takahasi probably doesn't want to keep doing the exact same worlds. He seems to have these ideas bouncing around inside his head for stories and settings he wants to do and fake people out before revealing it's actually tied to the greater series. 3 had the most obvious ties near the start, but clear he wanted to do a story about short lived child soldiers on top of being a Xenoblade game.
