CoC barely even works as a tabletop. The title itself gives away the villain of the session is either Cthulhu, Shub Niggurath, Yog Sothoth, Azathoth, Yig, Nyarlathotep, Hastur, Dagon, Mi-Go, Ghoul, Hound of Tindalos, Shoggoth or Brown Jenkin.

it works because it's not about combat which also limits what you can do scenario-wise. at best you're having a shootouts against mortal cultists, anything else and your loss of sanity will inevitably be the end of your character. strait jacket is the default ending in coc.
