Unironically open world games should be used to diagnose ADHD.

The only people who complain about open world game design are people who struggle with self-motivation, planning, task initiation, goal pursuit, and time management. Unlike other genres open world games aren't constantly dragging you by the arm along a preset path. People who complain about open world games being empty and boring are people who are incapable of forging their own path through life. This is the kind of player who will sink 5 hours into a game ignoring the quest log, ignoring 90% of the world map, running around the same part of the field in circles fighting the same three enemies over and over again and then complain about the game being "empty" and "boring".

Open world is just an interactive level select. Much like interactive cutscenes and interactive loading screens, it is targeted at people with executive dysfunction who can sink 5 hours into a game ignoring the quest log, ignoring 90% of the world map, running around the same part of the field in circles fighting the same three enemies over and over again and call the game an exciting way to "make their own fun".
Someone without ADHD could get more out of a game with an actually structured level select that can actually rely on holding their attention long enough to build up to something.

Most open worlds now are just filler with the same copy paste shit everywhere. They aren’t unique places with cultures and level design gimmicks (NV for example) that you can meaningfully interact with.
GTAV did not benefit from having the desert be that big with nothing in it

I literally have five of those six issues and play open world games just fine
In fact I think the top three help with the open world experience

The sole time I didn't like open world was Elden Ring because I ran into the same boss in seven different times and the game isn't randomized/proc gen so it had no excuse

Rain World would be much better at diagnosing this than what people typically imagine open world games to be. You need testosterone, IQ, and the natural urge of exploration to enjoy this game. The same urges to climb mountains, go to the moon and explore the unknown.
