Even most plotge feel the need to implement that framework, be it R18 titles or all ages castrations. The latter always feel like something is missing. Point: if your ge uses the heroine route framework and thus has romance as the main structural element, omitting the sex always means something is missing or rather has intentionally been removed. That's why the romance in key all ages ge feels so shallow. It feels like an important relationship milestone is missing. It's much more prevalent in VNs because they are written in first person, the intent is to self insert into the protagonist (which is why many MCs are faceless and almost all unvoiced). Fading to black and implying the act doesn't work here as it creates a disconnect with the MC, who has experienced a milestone in the story we were not part of. Anyway, essentially if you have romance you need sex due to the structure of VNs.
