It's based on the most populated region in Japan by a huge margin (more than Kansai, Kyushu and Hokkaido combined) yet because they've never expanded the region beyond the Gameboy map it feels smaller than Hoenn and Sinnoh. Celadon, Saffron and Vermilion in particular should be much bigger. Pokopia gave a decent hint of how a larger Kanto could look like.
