Most people these days don't start out with old FPS so that's not weird.Doom 2 on UV ups the ante some, but not harshly, it's a slow and steady escalation towards the end, you acclimate pretty easily.Plutonia plays harder ball, on UV it starts out as Doom 2 UV's endgame as a baseline, and then escalates pretty sharply from there-on.Doom and Doom 2 on NM is, naturally, hard as fucking balls, because official levels literally aren't designed for it, but people who get seriously comfortable with and proficient with the game can do it as challenge runs. Plutonia on NM is absurdly difficult and there's a single-digit number of people in the world who can marathon it that way.
