Which is why I said most, it also makes more sense in adventure games for this type of things to be mandatory. Both Journey to the West and Lord of the Rings had such caves and they are major inspiriations for adventure gaming (although of course the trope is rooted in real life as well); and it's the type of games where the thrill of the discovery is center to the gameplay

I haven't seen many irl waterfalls have you; besides 'realism' is irrelevant to good or 'bad design'. The fun is precisely to go beyond what one would do in real life, waterfalls and the small caves that get formed behind them irl invite imagination, and in the fiction we get to tap into that and fullfill the fantasy in a way you wouldn't irl. Same reason why in Duke Nukem 3D or whatever it's fun to enter the grocery's stores through the employee area and crawl through ducts to reach the next room, if you've never been to the mall and thought "I wonder how it's like behind that employee only doors" you're a boring person. Waterfalls in video games are like that.
