really fun game, at least the story was enjoyable

combat was braindead and once you get all the recover spells it’s a cakewalk but still enjoyable

does anything change depending on what god is selected at the start? wonder if it’s worth more playthroughs

The cutscenes Pious has talking to his master ancient are all dramatically different. For gameplay, certain ancient-aligned enemies will appear in different places and the ancients' runes will be obtained in a different order. It's basically like a difficulty setting of sorts. Also, there's a special bonus related to the story if you start a new playthrough after completing the game. You'll see the difference shortly, and it does matter to the story.

As >>12802119 mentioned, there's an extended ending after completing the game under all three alignments, and cutscenes directly involving the ancient Pious chose will differ. Some other cutscenes technically have a different version for each playthrough, but only aesthetically.
The main gameplay difference is the order in which alignment runs are found. If I remember correctly, the rune which is weak against Pious's master is always found first, while Pious's master's alignment's rune is always found second, and the rune which is strong against Pious's maser is always found third.
This matters for the recovery spell, because the Chattur'gha variant (recover health) is most useful and the Ulyaoth variant (recover magick) does nothing. What a blunder of game design, by the way. Mana is already free if you run around in circles, so why couldn't the recover-magick spell result in a net gain? There's risk in standing still to cast the spell, so there should be some reward. Anyway, choosing Ulyaoth as Pious results in earliest access to the recover-health spell, while choosing Xel'lotath as Pious results in most delayed access to the recover-health spell. Even this isn't such a big deal because the game provides limited-use healing items for all chapters in which the recover-health spell may be unavailable, but it's worth noting. Meanwhile, choosing Chattur'gha as Pious yields earliest access to the recover-sanity spell, while choosing Ulyaoth as Pious results in most delayed access to the recover-sanity spell.
For other spells, the alignment rune discovery order doesn't matter much, because only the effectiveness against monsters of each alignment differs, and Chattur'gha/Xel'lotath/Ulyaoth minions' alignments will be rotated throughout the game based on Pious's choice. Monsters of different alignments do have their differences though, particularly when it comes to zombies (with only Chattur'gha zombies able to regenerate limbs and so on).

There are also three versions of one of the bosses, but the gameplay is ultimately the same for each version of that fight.
The last thing to mention is that you should probably consider replaying the game if you never found the Mantorok rune, and maybe just look up how to get that if you don't mind spoilers and don't want to risk the frustration of missing it again.
No other missable content comes to mind, so if you got that then you probably saw everything except the unique cutscenes and alignment-rotated versions of each chapter's gameplay. Whether you should play the game two more times depending on whether you think you'll actually have fun doing it, rather than just trying to get to the end. If it sounds like a slog then I'd just look up the extended ending. Otherwise don't spoil it.

all i remembered with the Penny Arcade webcomic about the plot being real horroshowy.
sounds to me like a Cthulu game but with its own wacky obscure lore about going bonkers over inhuman gods controlling your MC's fate.
