Like you were once upon a time a people pleaser to the point of being somewhat detrimental to your own self, but the reassurance from those who left you behind helped keep a version of the world and yourself that was more tolerable. To me it sounds like you need to take some time away from others who make you feel good, live in that somewhat cold world that you fear alone with yourself for a few hours at a time, live in yourself for a while a time and really deeply wonder if that Disdain comes from yourself, or from outside of yourself, if that ugliness is you, or a perception that you hold of yourself. Distance yourself from people who tell you that it's always okay, that you are going to be okay, that you're good enough as you are. That is not to say leave them behind, but be mindful that things are not okay in yourself, that you're not okay with yourself. Then when you can accept that you're not okay, start looking for your best features. Lie to yourself a little if you really need to, and allow post-irony to take over. Let the little white lies become your law eventually if it does help. I don't know if I can really directly help, but it does sound like maybe you need a little time to yourself to kind of pick yourself apart, then stick it all back together accepting yourself piece by piece in order to love your whole self more. I've felt similar before.
