I think given Kaisex's experiences with Issho, and to contrast with Akane's capricious death, his death may be a malicious one who uplifts the doctor solely to bring him to the death that uh, the death, wants. Representing Issho's treatment of his students like pawns and his direct and specific humiliation of Kaisei.
Kaisei is going to tell a sad version and the whole ship will drown in sorrow. However he gets too deep into the story and starts crying as well and Issho will say a rakugo can never affected by their own story.

Kaisei's version of Death might keep the consequences of breaking his rules a secret as a test rather than out of malice, and only turn vengeful out of disappointment over his student's betrayal. On the other hand, Death's expectations are too high given human nature, so really he betrayed the main character first.If you look at Death God in the context of his other stories there's a steady escalation throughout. In the first the teacher is annoyed, in the second the cougar vows to haunt her boyfriend, in the third the debt collector shows up at the door with a sword, and now Kaisei's fourth story ends in death.
