Basically, Kyubey and his planet imprison Homura to see what happens if the Law of Cycles can't take over. As mentioned in the series, if Homura surrenders, her emotions will overwhelm her, transforming her into a witch. With so much time travel, she becomes a super-powerful witch. Madoka sneaks into the magical world along with Sayaka and Bebe, but only a part of her, to prevent Kyubey from seeing her memories and knowing who she is or gaining her powers. They know she doesn't exist as a person, so something strange is going on. Thanks to the three of them, the labyrinth disappears, and Megukami goes to take Homura, but Homura continues to blame herself and thinks she was responsible for everything, so she creates another labyrinth, trapping Madoka and the others, though it won't last long.

QB are aware of the Law of Cycles and want to to understand it so that put Homu in an isolation barrier stoppinfgher from fully transforming and consequentially not be saved by Madokami since she isn't a witch yet.

Homu's labyrinth is a fake Mitakihara and draws in those who are important in Meduka +/ Homu's story. Mami and Kyoko are both alive since died to witches (Bebe and Sayaka/Oktavia, Cycles preventing the existence of witches) Bebe+Oktavia are Madokami's celestial assistants since they already got Cycle'd in reality. Madokami is in there but stripped of her memories that she's a god since if she's still aware that she is Madokami the QB will be able to observe the phenomena of the Cycles and try to control her for more energy.

Homu discovers that things are off, she realizes that she's in a labyrinth but not that its HERS and memories of the real world slowly return and Sayaka helps Homu figure things out. After Homura realizes, she tries to destroy herself INSIDE the barrier so QB can't catch Meduka.

The other girls work together to break Homu out of the barrier from within bringing her back to reality and Madokami comes for Homu. In a conversation Homu had with Meduka while inside (which is real Meduka) Homu tells Meduka dreamt Meduka went away and no one remembers her, Meduka said she would never do that. To Homu that means as Madokami she isn't happy but still sacrificed herself. Homu tears the human part Meduka out from Madokami and rewrites the universe again to change Meduka's fate

In Homu's new universe, the Incubators are enslaved to clear curses. The girls live normal lives,Meduka doesn't know she is Madokami and gets to be a normal girl. Despite that near the end Meduka starts to remember saying that this isn't her true form and starts transforming back into Madokami but Homu hugs her and tells her that she is fine the way she is.
