He probably comes up with whatever on the spot, but he probably does have a general idea of where it should go. We see this from how he builds narrative significance through mundane details in each chapter. The real problem is the pacing. It's hard to notice, but the anime makes it more clear(at least for me). The story is being stretched thin. 2/3 of season 2 was like the cruise arc. You can argue the SSS party could have been 2 chapters too. Still, I think this slow pacing is also part of the appeal for me, but when I say this I don't think it really says much about the quality of the story being improved because it's stretched out, just that it's good that it's drawn out so I can zone out and turn my brain off while reading/watching.
