Tsukyiomi is not even close to the worst he can do. He didn't use the weakest technique because that'd be sus, but he also didn't use the best he had, he went for the middle ground, which is the perfect choice, sucks for Kakashi that Itachi's middle ground still packs a punch but he was literally fine after like 3 days.

He has techniques that deal more damage, but tsukyiomi is the best he can do when it comes to genjutsu and mental damage.
It is a lethal technique that can put the target at a vegetative state. Of course plot convenience wouldn't let Sasuke and Kakashi take that much damage, but it's still risky, you wouldn't use this on someone that is secretly your ally.
He could easily make the technique weaker btw, only the target knows what is going on, but he didn't.
