The main reason you don't see moderate sex in isekai is R15 magazines outright ban it while R18 there's the expectation it's porn. There isn't really room for inbetween.You do see inbetween sometimes though due to idiosyncracy with the editor, publisher, OG author and artist. Usually, the artist wants to draw porn but the author or editor won't allow it. Like with Isekai Zenra where the MC is literally naked the entire story and fucks his teenage wife within the first 5 chapters of the WN. Most likely what's happening is that the people who were fans of harem bait series back in the day are now writing but are sick of the baiting. So they're copying what they know, but going for an actual harem ending instead of leaving it unresolved, or picking one girl in the ending. They aren't writing harems because they can't imagine mono relationships, they're writing harems because they're sick and tired of "harems" that end with one girl winning and all the other nice girls are out of luck forever; or end with 'maybe the guy will do something with one of the girls eventually, but not during this series'; and want to see more actual harems and harem endings.
