Hammering in how he always put his sister first, and the reveal that his interests in monsters began from fantasies of wanting to help her, but then immediately contrasted with how he's failed her in multiple ways, both willing and unwilling, and how much it's been weighing on him. It all really comes across in that first panel, where you see him more serious, dour, and even somewhat sinister in ways he's never been throughout the rest of the manga.
