Presumably Rudy as a 40 year old man wouldn't be thrilled about being in a girl's body, even if he would eventually get used to it. It creates a source of internal conflict between what his male mind wants and is used to and his new physical form. It also likely helps dilute a lot of the criticisms about Rudy being an old man perving on little girls for a significant chunk of the story, because growing up as a woman this time would naturally change his whole reaction to such things and dramatically changes the relationships he would have with the other female cast members. It does complicate the story somewhat once you get to the parts that are about his harem family and popping out kids left and right, unless there is a 'grow a dick' spell AND he manages to woo his wives even as a woman.

In a better story, Rudy's past and disgusting nature would be obstacles that he has to overcome. And we get a BIT of that, which is really well done! But all it really means is that he goes from an agoraphobic and introverted disgusting pedo to a traveling and confident disgusting pedo. He only ever confronts his past in ways that make him 'cooler' and improve the power fantasy, he never has to change his ways such that it would conflict his with base and hedonistic urges.

There isn't a single isekai that handles genderbending well. The best I can think of is Youjo Senki, where the protag being female is completely irrelevant. The MT author is so sexist he made souls have genders to specifically rule out genderbending.
