Telling him about his appearance depends on if it's about something he had from the start when you met him, or developed later. Like getting fat. But if it's something he cannot really help, like balding, or he had from the start will make you look very shallow.

I mean it depends. I understand how the attraction can change a lot for you. But of course if he is also somewhat insecure about balding, then you could just support him and tell him to help him. Most men would want to not get bald if they could. But the effort to deal with it (the cost, having to take meds with possible strong side effects). Maybe it would help if you do all the research for doing it. If you dont say anything at all, it won't get better and you probably will leave him sooner or later.
