Why do I sometimes slurr my speech when talking? Do I have parkinsons? Did I have a stroke? I remember sometimes the side of my head like around my temples would pulsate and hurt a bit but it doesn't do that anymore.

Your mouth has likely just gotten lazy. Spoken word and enunciation used to be a rigorously graded skill. Practice some readings now and then.
Or could be a gradual physiological change, like jaw misalignment or slightly swollen tongue. some deficiencies can cause the latter. Anemia I think, among others.

I see. Thing is, it only happens when I talk naturally, not when reading outloud.

Mind moving faster than your mouth.

I forced myself to not use specific words, and the puzzle slowed down my brain so my mouth can say it.
Specific words being: the basic way I want to say it. Gotta say it differently.

Unintentionally improved abstract communication too, because I can rephrase faster, which helps when someone doesn't understand the first thing I said.
