How do you tell the difference between someone whose brain was fried on drugs/alcohol and someone who just had a head injury of some kind?

substance people have more trouble with memory and social intelligence like talking to themselves in front of everyone. TBI people have deep cognitive decline, like they have a hard time forming sentences and or cannot finish one. They have extremely slow processing speed. They cannot abstract think or do abstract problem solving.
