Since the end of my long term relationship I've felt an overwhelming need to self harm and punish myself for the mistakes that led to it.

How do I overcome these thoughts? How do I forgive myself?

By focusing on what you can change in the future relationships and forgetting what you did day by day. Take it as a learning experience and now you know what not to do anymore. Eventually it will hurt less and less.
