If you need a step-down program, stop watching hardcore and switch to softcore images, then switch to audio, then erotica, and finally quit that as well.
Here's some pseudoscience: Porn gives your brain an unnaturally fast spike in arousal, from 0-100 in a few minutes, provided entirely externally. You want to slow that down and make it come from within, and you do this by making the stimulation come via imagination. Hence, shift to softcore to wean yourself off the heavy stuff, then to material that requires you to imagine to get the full experience (audio and erotica). Finally, use your imagination only.
You'll have intrusive thoughts. Porn is unfortunately a virus that burrows itself in your mind. But when you have them, either shut it down or shift it to healthier sexual thoughts.
And give it time.

Do a radical change in your daily routine and environment. You only watch porn because a certain trigger tells you to do so. If you find and eliminate that trigger, you will stop doing it. I have pretty much the same story as you. From time to time I average on about 16 jerks per week. Thats pretty much. But every time I change environments, and thus loos all the triggers that trigger my gooning session, I stop to even think about it. Once I moved for 3 months to a different city. I had no problem to not cum for the whole 3 months. When I moved back, I relapsed in 1 day. But I actually stopped trying, because I didn't see any benefit. Maybe I had a little more time in the day, but thats it.
But if you want to stop, change your routine and, if possible, your environment. Most people probably jerk off when they are bored or something. So try to be as active as possible. Make it a habit to go on a walk in the evening. Go to the gym. Create an routine that doesn't offer the time frame for you to nut.

The thing about addictions (like fapping) is that it's not done out of hedonism. It is done as an instant mood corrector. You are feeling anxious, depressed, lonely. You fap, you feel better.

You never develop healthy coping skills. You never decide "I want a girlfriend, I'd better work out". And just fap your troubles away. You never decide "that school project is stressing me out because the deadline is getting close and I haven't got started", you just fap to feel relaxed. So your entire life becomes dealing with stress by fapping while problems pile up instead of addressing the causes of the stress.
