Everybody smoked cigars and pipes before cigarettes became popular, it's just that lots of people switched from pipes to cigarettes. The first surgeon general's report on smoking confirmed this, as did similar studies in Canada and Britain. Subsequent surgeon general's reports avoided this embarrassing complication by mixing cigar and pipe smokers in with cigarette smokers handwaving away the differences between them as inconsequential.
