In school. Teachers stopped enforcing etiquette. After leaving school you'd go out into the world and people would expect you to behave properly and you'd expect the same from them. So you would. My friend is a high school teacher, and from what she tells me, all they care about is making sure the students aren't asleep (when a staff member walks by the classroom).

What I'm talking about is this phenomenon where people who walk into a restaurant or bar, ask for a cup of water (similar to what OP is talking about in McDonalds), and by law the establishment has to give to to them. I can't remeber where this was, maybe Texas, maybe some other states too, but I remember reading about it being a thing.
