The show literally tells you it isn't real and just Dexter's way of interpreting his trauma. It's only the books where it's implied that the dark passenger is actually the spawn of Molach. In the books Doakes had a dark passenger but he didn't know anything about it and satisfied it through killing people as a cop, he hates Dexter because he could sense his dark passenger and instinctively knew something was off about him

So yes Dexter actually has a demon entity in the books but they never explore that aspect afterwards.Also in the books Doakes gets gets legs and arms cut off and also his tounge
