I'm guessing the joke is cats like milk, so Heathcliff would get a cup of milk instead of coffee at a diner. But the waitress isn't looking at him, there's someone else facing the viewer (I first thought she was talking to the man) and so much of the frame is random drawings.I do like this strip for how absurd it is, but I don't think the joke is well done.
