1) He's too much like bystander to his own big moments. George Stacy? He didn't actually attack him. If This Be My Destiny? Left his base to collapse on Spidey. Sinister Six? Inherently diluted by being part of a team.
2) As a portly scientist, he's inherently a bit goofy to the average reader, and he's not the right kind of insane that works off of that. Like they actually played with it a bit in that one comic series, I forget the name of it. The one where it's like, minis that revisit the origins of Spider-Man villains and redoes them. Otto is teased there as being the kind of like, detached, demented, slovenly, indulging mad scientist, but then always reveals that no, Otto is just disturbed and downtrodden. He'll lash out and attack but he's not that kind of evil.

He's not that cool looking for a film. He's a relatively ugly little middle age guy in a radiation suit with robot arms. Meanwhile Green Goblin, Venom, Hobgoblin, they have iconic looks that directors love.

Plus he was a nobody in the 90s. Dock Ock fell off pretty badly for a while and no one cared about him since he was not one of the extreme cool villains.

He was a joke like most 60s era villains in the late 80s to late 90s. Most were killed off and replaced by cooler more extreme villains that wore leather jackets and had asymmetrical costumes.

Wasn't just Spider-Man, most villains in that era were replaced or updated. Just about every GL and Flash villain was replaced by a new guy at that time. Half the classic Batman villains were considered lame old silver age relics too.

I'm more of a Norman Osborn guy but even I know that's not true, Octo had a great early 90s with the white suit and the Sinister Six revivals and his backstory getting fleshed out, he was doing great until Kaine killed him. He was dead for almost 3 years of the 90s, and most of his stories after his return sucked because Spider-Man comics in general tended to suck by the later 90s.

He arguably had a worse 1980s, with years spent with a crippling phobia of Spider-Man, probably so they could push Hobgoblin as the new hotness, and by the time Ock finally gets back to normal, Venom is introduced right afterwards.

Beau DeMayo is a weirdo but nothing has contradicted what he said about how Feige and the whole "tacticool" pseudo-military aesthetic obsession to make the comic designs look less 'lame' in real life.

Even recently, Morrison called out Lindeloff for mocking "Green" as "stupid" because those idiots think the reason the Ryan Reynolds film flopped was because there was too much 'green' and not the shoddy CGI and questionable plot.

Yeah, and I'm honestly surprised that angle hasn't been pushed more. It's something that would help both Otto and Peter. It paints the former as more of someone that genuinely was respectable and had been twisted beyond belief through abuse, paranoia, accident, neglect, and so on, and it gives more credence to the latter trying to understand his villains more and hoping for their rehabilitation.

But also in general I always like how Otto has been a bit of a tragic villain that people can sort of reach out to, whether it's him agreeing to help Richards (for as little as it paid off) in Fantastic, or acquiescing to help Peter let civilians caught up in the fighting escape (even if he then was content to allow the same building to drop on Peter). I will still never forgive Wells and Slott for backsliding him after Spock Vol 2 and Superior Four, when he was otherwise almost finally about to properly settle into his role as an Anti-Villain
