Reminder that if you can clearly see down to your scalp when your hair is wet, your hair is thinning, lots of small new hairs sprouting at the hairline or crown is a sign of hair recession.

Finasteride/Dutasteride are 5-alpha reductase inhibitors that prevent free T from being converted to DHT, a byproduct of testosterone that serves no purpose after puberty but to choke out your hair follicles and make them thin out and die in a few short years, a condition known as androgenic alopecia.
5ar inhibitors prevent your hair from getting any worse, mild regrowth is expected, rapid onset diffuse thinners see a significant recovery on fin alone. Can be taken orally or topically.
5ARIs are not dosage dependent, taking as little as as 0.2mg daily fin can reduce serum DHT by 68%, while 5mg reduces by 72%.
0.5mg daily dut reduces serum DHT by 95%, but is considered a second line treatment, and can be more expensive and harder to get prescribed. Start with fin.

Fin has been used to treat benign prostatic hyperplasia (BPH) for 30+ years, it's very well studied and is tolerated well in almost all men, however, there's a 1-2% risk of erectile issues in controlled studies.
These can be avoided by reducing dosage, spacing out dosage, or stopping the drug. Fin takes days to fully bio-accumulate, the half life of fin is about 8 hours, adverse effects occuring within minutes of starting fin or persisting long after stopping fin are not real and have no basis in reality. Please note that dut has a much higher half-life of 5 weeks, if you're prone to fin sides, dut may cause them for far longer.

Min is taken with fin to restore your hair to it's original hairline, thickness and vigor when hair loss is caught early enough, (pre-NW2). Can be taken orally or topically.

Topical min does not work in 1/3 of men because they lack the required enzymes in their scalp, all men have this enzyme in their liver so the oral route is a good fallback.
The FDA formulation for 5% liquid minoxidil is food safe (vape juice, ethanol and water) and can be safely taken orally if oral min tabs are expensive or rarely prescribed where you live. As a general rule, ~4 drops (0.1ml) = 5mg minoxidil.
There's a 1-2% risk of fluid retention with higher oral dosages, start with the lowest dosage possible and work your way up when you're sure it's well tolerated.

Ketoconazole is an anti-androgen and antifungal that inhibits DHT on the scalp and used to treat fungal infections of the scalp that may be making your hair loss worse such as seborrheic dermatitis, a condition where you exhibit severe greasy, flaky large dandruff flakes with dried yellow crystals (blood plasma) embedded in the scalp. Some people may only have traction alopecia (hair loss from scratching or friction), but it will not cure androgenic alopecia on it's own. Can only be taken topically.

Do not use spirolactone. Spirolactone is a diuretic which, in large doses, inhibits testosterone and, by extension, DHT.
It is regularly prescribed as an anti-androgen for male to female HRT, and thus, comes with an astronomically higher risk of sexual side effects in men including ED, gynecomastia, and can cause potassium toxicity.

Not the best comparison pics admittedly due to the different angles/light/my longer (and greasy) hair in the new pic but very happy considering in the last pic I tried to part my hair as much as possible to expose any bald/thin spots

1st pic was my bald spot that was forming while I had similar length hair to the final photo, 2nd pic is me buzzed down right before starting my stack, 3nd pic is slightly after starting, 4th pic is my first dread shed and 5th is now 10 months later

I can really spot the difference myself when in downlighting and coming out of the shower

Just under 10 months of topical fin and min and I've been slacking on my microneedling and even my dosages amount (am poor atm so been doing half dosages for my topicals to get them to last longer) so hopefully I can squeeze out a little bit more gainz to get closer to baseline next year

The short hairs facing downward used to be on a patch of scalp that was completely hairless. Well, hairless but with those tiny normal skin hairs. There are multiple kinds of alopecia and finasteride doesn't treat all of them. Finasteride is only for androgenic alopecia. Diffuse thinning is more often associated with telogen effluvium, alopecia areata and other conditions that aren't caused by dht/hormones. you don't need to and shouldn't apply it in any of the dht resistant zones of your head. if you are missing hair in any of those areas you need to see a dermatologist and get diagnosed because if you have any hair loss in dht resistant areas it means you have a completely different type of alopecia that fin/dut don't do anything for, and you're taking the wrong meds. minoxidil does work for some of those and not for others. if you have hair loss where you shouldn't then you need a diagnosis and then you should take a holistic approach to treating it.
and if you do have hair loss that looks like it isn't male pattern baldness -- its common in men who have any other form of alopecia outside of male pattern baldness, to also have male pattern baldness at the same time. just because it looks like you have one doesn't mean you have both.
i had a scary amount of hair fall out but because it fell out around my ear i knew i didn't have male pattern baldness and with a quick diagnosis i was able to get all my hair back without taking fin/dut just minoxidil vitamins (especially d/k2mk7) and better hygeine/sleep/lower stress/etc. i know one guy and two women irl who had diffuse thinning that i urged to get nutrition/mineral panels done and all 3 wound up being low or high iron.

Earlier today I got a trichoscopy and I notice a few oddities.

>medium-low hair per cm2
>normal follicular units per cm2

How can this all be? How can I have a normal amount of follicles (80-100 is considered normal according to google), but have such a reduced hair count? And why are there so little vellus hairs?

I have a theory, I started dut and min around 3 months ago, so I could be in a shedding state that kicked out all the vellus hairs, but shouldn't the follicles still be visible?

Yes, but not in the quitting sense. You need to shave it or keep to short to treat it. I also see you have a topical infection of sorts or psoriasis. Shave it, clean it, treat it, massage that. Even if it’s not by much, it will at least regrow a little nicer.

Just got back from the dermatologist. I've been on fin since march, topical, applied every single night without missing one day. They were so amazed by my regrowth that my pics are going on the before and after page of their wiki. I have not done the medical release paperwork shit to post it but this is an approximation of my situation, hand on my heart, not lying.

Not mine just something I found on the internet, but pic is about where I'm at in terms of density, but the hairs on my crown are much shorter as this is a 2 years progression pic.

I tried gradually lowering my topical fin dose years back and after a few months, my gains started disappearing.
I think that either low doses of finasteride aren't as effected as the studies would have you believe, or applying topical finasteride naturally leads to microdosing in individuals who don't shave their head because a good portion of the topical solution doesn't reach the scalp even if you're careful with it.
