I'd just like to interject for a moment. What you're refering to as MOLLE, is in fact, PALS/MOLLE, or as I've recently taken to calling it, PALS plus MOLLE. MOLLE is not an attachment system unto itself, but rather another load-bearing system made of a fully functioning PALS system made useful by the PALS attachement grid, pouches and vital system components comprising a full webbing system as developed by the United States Army Natick Soldier Resarch, Development, and Engineering Center. Many grunts run a modified version of the PALS system every day, without realizing it. Through a peculiar turn of events, the version of PALS which is widely used today is often called MOLLE, and many of its users are not aware that it is basically the PALS system, developed by the Land Warrior Program. There really is a MOLLE, and these people are using it, but it is just a part of the system they use. MOLLE is the load bearing sytem: the gear in the system that allocates the grunt's resources to the other equipment that you run. The load-bearing system is an essential part of an operator's system, but useless by itself; it can only function in the context of a complete gear system. MOLLE is normally used in combination with the PALS webbing: the whole system is basically PALS with MOLLE added, or PALS/MOLLE. All the so-called MOLLE distributions are really distributions of PALS/MOLLE!

"MOLLE backpack" is a category so large it's borderline meaningless. It includes surplus issue packs; the whole range of commercial "tactical gear brand" bags from pretentious larper shit to actually useful and battle-proven specialist gear; the whole range of chink knockoffs of the former group, from absolutely retarded cheap shit to fully reliable 1:1 repros at half the price; a newer and smaller subgenre of "grayman" stuff that focuses more on velcro real estate for obnoxious "morale" patches; and, of course, the absurdly overpriced, overengineered and overhyped streetwear accessories with out-of-spec PALS grids and buzzwords like "modular" and "tactical" in the description.

Chief reason for their popularity is probably the sorry state of civilian backpack market, where long-term comfort comes third and durability is an afterthought. A real mil-spec backpack will outlive 2-3 similarly priced and sized civilian counterparts, especially under hard use (as opposed to "laptop+book+change of clothes" nominal payload most urban backpacks seem to be built around). However, the tactical gear companies are seizing this opportunity pretty hard right now, and coming out with packs designed and bulit for a modern urbanite's lifestyle, but with materials and details reflecting lessons learned in MIL\LE space.

As for the majority of "MOLLE backpacks" on the market, the general advice is "stay away". Unless you're absolutely sure you need to hang extra pouches off of your EDC backpack, you should keep to slicker urban designs just because of footprint and snag hazard.
If you ARE going to hang pouches off of backpack, you should still prioritize slicker negative attachment systems like molleMINUS, and try to keep pouches color-coordinated to the pack. On that note, stay away from camo patterns, especially the current or recent-service ones like multicum, ATACS, UCP or M81 woodland. Leave those to the servicemen and the LARPers they despise, and stick to solid colors like grey, black or tan.
