So he had to keep repairing the tissue as he aged. Every subsewuent surgery post-Thriller is to repair the structure of his nose. If you really look you will see he often added cartilage and tissue to make his nose bigger, and you can see his nose peeiodically go from small to bigger every couple of years. It's the same reason why the burns on his scalp never healed and remained painful for the rest of his life. During flare ups his skin would literally eat itself.
