3 things: eat, sleep, train

if you are not growing, then review each of these. assuming you are eating and sleeping sufficiently, do review your training. are you performing with proper forms? do you feel ur muscle fibers? is your ROM intact? are you sure you are stretching and contracting sufficiently rather than throwing weights? try training at different tempos slow or fast, and determine what works for u.

if none of these work, do consider your age, blood work panel, genetics, etc.

This isn't exactly true, but it's 99% true
Hyperplasia of muscle can occur for example if you take roids, or if you do very high level training - though I assume you're a functioning male with T in your blood and none of this is really a concern

Definitely false, muscle is actually one of the places the body prefers not to put scar tissue because it has to be usable

Look at roiders hands they get way bigger into training and it looks odd compared to their frame/forearms and is a softer bone that takes less load and nobody trains shit that makes hands denser. Youll also notice their frames grow horizontally, ronnie didnt have the same frame at 18yo even though he was roiding super early.

calf raises are retarded - will fuck up gastrocnemius because it's rubbing on bones of knee joint (the pain in back of knee when u try maxing out calf raises), gastrocnemius bends the knee u need knee bending movement to grow gastrocnemius, not knee extending movement
