That's a consequence of lifting with too much intensity all the time. If you go for heavy 5RMs or 3x5's you're more busy with getting the weight up than performing a technique that is safe and feels comfortable. It's often more beneficial esp once you get stronger to do 5 sets of 3 at 80% instead of a 3x5. Systemic wrong technique is also an issue but it can be prevented by videoing yourself from time to time and looking into the things that are causing your poor technique.

When you're lifting submaximal like in a 5x3 @80% you also notice that something is making you hurt without immediately getting an injury.

I've never done a hack squat because none of the gyms I've gone to had one, but I imagine it gives a false sense of security because noobs don't learn how to be stable and brace. Weight is still on your spine, with a barbell squat you quickly learn to brace and be stable and coordinated in general or you fold over like an asshole.
I am 33, I lifted heavy at ages 17-20 and took it up again at 30. I'm at 1.5/2.5/3.5/5 and I've practically never had lower back pain in my life, I've injured other stuff but never my back.

Jefferson curls and back extensions are the best. Low back injuries are from neglecting isolated lower back movements. We do isolation work on every other muscle on our body, but there are zero routines out there with any low back isolated movements. Deadlifts and squats are big, compound movements. When those are done correctly, they aren't putting the low back through any range of motion under tension. They are a static hold. Static holds don't build strength and size like moving the muscle through it's entire range of motion while under load.

And it makes sense, humans aren't really built for sports of any kind. I'm not sure I believe this. Working out builds muscle and muscle makes is strong, and while I think there's merit to the idea that many people exert themselves excessively and accumulate wear and tear on their bodies in the process, to completely forego building muscle to me seems to be setting yourself up for failure. Probably next to no one benefits in the long run from being a bodybuilder but who wants to be weak? There's a middle ground here that we should pursue.

Any action, no matter how easy and simple, you'll eventually fail given enough repetitions since your brain's signals always have some amount of error. With weights the problem is that the inevitable failure can injure you. Compound exercises require a lot of muscle coordination, if something in that chain is not firing for whatever reason, the whole movement can be compromised and the tension required to protect your spine etc. just will not be there and you'll fold under the weight.
