How hard would it be to design a small raspberry pi computer with a touchscreen running Linux, that has a camera, bluetooth, and can connect to cell service? Surely it can't be that difficult, unless I'm missing something.

Yeah, I know Arm is preferred for mobile but it's utter dog shit for an open source ecosystem that actually has to be useful and support modern features. Using it in the project means certain failure unless you have a team of firmware and software engineers.

Modems are insanely sus due to their government enforced black-box nature. It's likely illegal to modify the firmware due to how brittle the cell network is. They have to be isolated from any bus that has DMA.

considering you can just straight up run linux on an rpi trivially and get a WWAN card you can pop a SIM into and then get a number through a VoIP service, it should be pretty easy to build but still likely not worth it since if you're that paranoid about glowies on your phone, you're likely to just not use a phone at all

postmarketOS exists. Does it have to be Linux though? AOSP is open source, you could build an open source custom rom for a device like that. Having it run on a Raspberry pi doesn't sound like a good idea though, it's better to pick hardware more suited for the job.

You can simply buy a very small laptop or similar small computer running a normal OS, and then get a VoIP service.

They're listening to everything you do over 4/5G service anyway, whether it's voice or data. In fact, modern cellphones basically operate as VoIP devices and your "voice" call is encoded as audio data through a proper codec and routed through the internet to get to the recipient.

It just reminds me that I don't even want a phone number. I only have one because everyone denies me services over not having one. I have received less than 30 legitimate phone calls in the past 10 years not counting personal contacts. But the mere existence of a 10-digit number that anyone knowing gives them unrestricted access to my anus (voicemail) shouldn't be tolerated in the current year. And the fact that other communication platforms build their ID on top of this system is totally baffling to me.
