no problem. this sort of lora is actually really, really easy to make once you know how. it's quite literally a 15-17 minute config on a 12gb 3060.

Could be several things. Your denoise is too high, which introduces too many changes. Your upscale factor is too high, which results in the model having to segment your image and interpreting each segment separately, trying to squeeze the entire prompt into each. These two compound. It could also be that you're just not specifying the number and arrangement of characters, which causes the model to interpret potentially "contradictory" tags (like different expressions) as belonging to different subjects - and subsequently adds those subjects to the image.

Like the other anon said, adetailer is just automated inpainting and hiresfix is just img2img upscaling. The problems with them are:

Yep, pretty clear cut. You're not specifying enough and then giving the model a ton of pixel space with a fairly busy backdrop, which ends up with it trying to guesswork the background blobs using your existing tags as a crutch. In addition, 2048px is also a bit much for SDXL to do in one step. If you slice the picture up into 1024x1024 squares (SDXL's training resolution), you can kind of visualize what went "wrong" there and why the girl in the middle appeared.

In short, lower your upscale factor a tad and tag more to guide the model away from leaning on potentially inappropriate positives.

The latest is that I tried manually adding only quality prompts to all the positive and negative prompts for both hires fix and adetailer, so that they aren't using the base prompt, and now I can reliably get 2girls without extra characters that way. This is at 0.55 denoise.

Oh, you were using ADetailer? That wasn't in the metadata. Yeah, it'd be another explanation. FYI you can edit the sensitivity of ADetailer's detection model under detection settings. For faces, 0.7 or even 0.8 is more than enough. Similarly, you can change its denoise strength and so on. Generally, ADetailer doesn't need much denoise but benefits from higher resolution (as does all detail-inpainting).

Detection model confidence threshold. Keep in mind that, even if you do change it, it'll switch back to default values upon webui restart. Very, very common problem, since anime models have an obvious and understandable bias toward 1girl. Tag more! Specifically, tags related to interpersonal actions or arrangements, like side-by-side, holding_hands and whatever else. Concepts that aren't feasible with a single subject = model will gleam that there's supposed to be more than one in the composition. Stuff like this is a bit like a language. You pick it up.
That said, it may also be that your specific mix/tune is completely fucking cooked on males, because that, too, is a thing that happens. Can't tell with confidence, as I haven't used your particular one, but it's a chance. Small one, but real.

The way something is drawn can depend on style used. For some styles you may be able to get rid of some things only with negpip, while for some other styles you will never encounter the thing in the first place.

If you don't already know what you like, that's a terrible way to know what you would like. Premade mixes are biased towards a style so whatever style tags you experiment with will just be a diluted version of what it should be. And if you're experimenting with loras, which is what you should be doing given how many styles there are that base models don't know well, then it's again better to do it on top of an original base model where there is less dilution of the desired style. Especially given that most loras are trained on the base model, not a specific shitmix and especially not an obscure one.
