Most creatives just want a "please do this, here are some references kthx" format. When you start mentioning other characters' body types or scenarios such as combat damage (or even spinal tilt!?) you risk confusion because you're adding details not relevant to the design. Ideally you want an artist you can go back and forth with, especially since your demands are so specific. There's always some wiggle room between what is in the mind's eye of the commissioner and the skill of the artist so you need to allow for that.
